The working view. Step 1: click Open In-Progress list (opens PCS in a new tab — the Harvester panel appears there; run it and copy/download its output). Paste last week's harvester output and set the date it was run. Anyone still outstanding now will show as outstanding since that date. (After this, the tool tracks it automatically on this machine.)

Not part of the weekly run. The Matrix covers completed forms — including signed-off incidents. Once signed off, an incident can't be edited, so a witness listed here without a statement is a permanent gap. Use this view to measure that (e.g. how many sign-offs went through with statements missing), not to chase.

Staff initials — exceptions (when someone signs with 3 letters, e.g. Haddy → HMD)

Most staff sign with their initials (first + last). List anyone who signs differently, so the tracker matches them. Comma‑separate if they use more than one (e.g. HMD, HDM). Saved on this device; the built‑in list ships to everyone.
